Headlight offense happens when you are driving around with only your parking lights on (lights button only pressed once) when you should instead use low beams (lights button pressed twice). If you were fined you probably only had your parking lights on, as the game will not fine you if your low beams are on.

From what i can tell most US states require headlights on from half an hour before sunset to half an hour after sunrise.
